Clinical Pharmacist - Medication Therapy Management

Location: Naval Branch Health Clinic - Mayport, FL

USGP is seeking a qualified and motivated Clinical Pharmacist to support the Medication Therapy Management (MTM) Department at the Naval Branch Health Clinic in Mayport, Florida. Position Highlights

  • Schedule: Monday - Friday | 0600-1800
  • Pay Rate: $76.00/hour + Health and Welfare Stipend

Minimum Qualifications
  • Degree: Doctor of Pharmacy (Pharm.D.)
  • Education: Graduate from an ACPE-accredited college or university
  • Residency/Fellowship:
  • Completion of a clinical pharmacy residency or fellowship accredited by ASHP or ACCP,
  • OR  documentation of equivalent clinical education and CME in clinical pharmacy
  • Experience:
  • Minimum of 12 months of experience within the past 3 years providing direct patient care in an ambulatory clinical pharmacy setting
  • Certification:
  • Current Basic Life Support (BLS) through the American Heart Association
  • Licensure:
  • Active, unrestricted license to practice as a pharmacist in any U.S. state or territory
Immunization Requirements (Must be current or willing to obtain):
  • Hepatitis B (series and titer)
  • Tdap (Tetanus, Diphtheria, Pertussis)
  • MMR (Measles, Mumps, Rubella)
  • Varicella (Chickenpox)
  • TB (Tuberculosis) Test
  • Seasonal Influenza Vaccine
  • COVID-19 Vaccine (if applicable)
Key Responsibilities

Responsibilities include, but are not limited to:
  • Evaluate and manage patient pharmacotherapy in collaboration with medical staff
  • Initiate, modify, or discontinue medications for chronic conditions (e.g., diabetes, hypertension, asthma) under clinical protocols
  • Interpret and evaluate lab results to support therapy decisions
  • Order therapeutic nutrition products and adjust drug therapies based on patient response
  • Review patient profiles for interactions, contraindications, allergies, and therapeutic duplications
  • Provide direct patient counseling on medication adherence, self-monitoring techniques, and compliance aids
  • Utilize EHR systems for medication documentation, tracking, and communication
  • Administer or prescribe medications per established agreements and protocols
  • Document and report adverse drug reactions (ADR) and medication errors in accordance with TJC or AAAHC standards
  • Participate in medication utilization reviews (DUR), quality improvement projects, and staff training initiatives
  • Provide clinical consultations, patient assessments, and support for disease state management and polypharmacy
  • Educate patients and staff on medication therapies, safety, efficacy, and emerging pharmaceutical trends
  • Support interdisciplinary health teams with therapeutic recommendations and contribute to care planning
  • Maintain knowledge of pharmacogenomics, pharmacokinetics, and complex medication regimens
  • If certified, may perform auricular acupuncture as permitted by DHA and MTF policies
